Ischemic mitral regurgitation and its related risk after myocardial infarction.

چکیده
In this issue of Circulation, Bursi et al 1 report that ischemic mitral regurgitation is a more common finding after myocardial infarction (MI) than was previously thought and is an independent predictor of heart failure and death among 30-day infarct survivors. Seldom is mitral regurgitation actively investigated, and investigation is often only prompted by clinical examination. The data Bursi et al report support findings from the 1970s, which suggested that clinical examination is not sensitive in the detection and assessment of mitral regurgitation.2 The authors conclude that Doppler echocardiographic assessment of mitral regurgitation should be actively sought and included in post-MI risk stratification.

Echocardiographic assessment of ischemic mitral regurgitation
Ischemic mitral regurgitation is an important consequence of LV remodeling after myocardial infarction. Echocardiographic diagnosis and assessment of ischemic mitral regurgitation are critical to gauge its adverse effects on prognosis and to attempt to tailor rational treatment strategy. There is no single approach to the echocardiographic assessment of ischemic mitral regurgitation: standard e...
متن کامل[Role of echocardiography in the detection and prognosis of ischemic mitral regurgitation].
Ischemic mitral regurgitation (IMR) is mitral regurgitation (MR) due to complications of coronary artery disease and not fortuitously associated with it. Acute MR secondary to ruptured papillary muscle after myocardial infarction is rare and often fatal. We focus on functional MR, much more common, which occurs without any intrinsic valve disease. It was often underrated because of low murmur i...

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...
متن کاملLeft Ventricular Mechanics in Functional Ischemic Mitral Regurgitation in Acute Inferoposterior Myocardial Infarction
UNLABELLED Ischemic mitral regurgitation (MR) is an established adverse prognostic factor after myocardial infarction (MI). Functional ischemic mitral regurgitation in acute phase of MI remains under-investigated due to its often transient and dynamic nature. We aimed to assess left ventricular (LV) mechanics by speckle-tracking echocardiography in acute inferoposterior MI and ischemic mitral r...
متن کاملIschemic mitral regurgitation
Ischemic mitral regurgitation (IMR) is a relatively frequent complication of myocardial infarction. The presence of IMR negatively influences imminent mortality after myocardial infarction and the long-term survival of the patients. IMR is a functional, not morphological impairment of mitral valve and is caused by a change in the geometry of the left ventricle. In the review article, the author...
